(The following article was written by David Buchanan, Head Coach at Mason County High School in Maysville, Ky. A similar questionnaire can be adopted for your program. Coach Buchanan can be reached at david.buchanan@kyschools.us )

* Turn in to Coach Buchanan at the beginning of practice on Monday!

As a football player, you want to be a low maintenance player. High maintenance players require a lot of time and attention by the coaches to get them to do the things they are suppose to do. College coaches generally shy away from high maintenance players- they want to focus on coaching football, not holding a player's hand.

This questionnaire is an instrument to help you measure your maturity at this point and maintenance level. Low maintenance guys are also team first guys- this will help you see what kind of teammate you are!

We will do this again at the middle point in the season, and at the end of the season. Your score at the end of the season, agreed upon by you and Coach Buchanan, will be reported with your video to college coaches.

This questionnaire will help make us a better team, you a better teammate and player, and prepare you for some of the harsh realities of college football.

There is no right or wrong answer- be honest with yourself so you know where you stand!
